1. ASSISTANCE TO INDIVIDUAL IN CRISIS SITUATION (AICS)

This is a DAILY service/intervention of Emergency Assistance Program to be extended to a walk-in needy adult client who comes to the Office requesting for financial assistance for his/her immediate need such as food, medicine and transportation.

 

REQUIREMENTS

1.    For Food : Certification from the Punong Barangay that family head/breadwinner is sick
2.    For Medicine :   Certification from the Punong Barangay that family head/breadwinner is sick, Prescription of Medicine with letter head of a government hospital/clinic
3.    Transportation : Certification/endorsement from the Punong Barangay

  1. The client proceeds to the Treasurer’s Officer for the release of cash assistance.

2. PROVISION OF SENIOR CITIZENS  AND PERSONS WITH DISABILITY IDENTIFICATION CARD

This is a DAILY intervention of Special Social Services for Special Group and disadvantage group such as senior citizen aging 60 years old and above and persons with disability who wants to avail their privilleges per RA 9257 and RA 9442 respectively.  This service is  free of charge.

 

REQUIREMENTS

a.    Person with Disability
- Application form certified by MHO
- 2 pcs – 1x1 picture

b.    For senior citizen
- Duly accomplished application form
- Birth certificate /baptismal certificate/voter’s affidavit
- Residence certificate
- 2 pcs- 1x1 picture

3. PRE-MARRIAGE COUNSELLING

This is a service under Family Welfare Program to be implemented to would-be –couple aging 18-25 years old who wants to get a marriage license. If one to the partner is above 25 yearls old who still wants to undergo the counselling.  

The legal base for this service is base on the New Family Code.

 

SCHEDULE:

Schedule for Marriage Counselling is twice a month (First and third Wednesday of the month) to be administered by accredited Counsellor.
